  1. Only device association is needed for registration. Setting the primary extension results that the extension gets the Directory URI of the user. Primary extension can be set after a device got associated to the user.

  2. it should be question with two options. CAPPS Foundation Learning Guide on page 290 says:

    Follow the list to set up a Cisco Unified CSF device for Cisco Jabber in softphone mode:
    ■ Configure the end user:
    ■ Create and associate the new CSF device. (E)
    ■ Set the primary extension. (A)
    ■ Enable User for Unified CM IM&P.
