From my understanding it’s not asking what you should choose, the question is “Which should be upgraded”, with 4000 endpoints SQL 2008 Express R2 Server is enough, as we are bypassing 5000 limit, then we would need to upgrade SQL database from Express to Standard, therefore the answer should be D.
Maybe it is D as you wouldn’t upgrade Mirage Management Server you would add another one.

Here they are talking about “Mirage Gateway” not “Mirage Management Server”. In the bottom of pg 16 they say:
Number of Management Servers
In most cases, two Mirage Management servers provide sufficient availability and resiliency for a successful
deployment.
So from my understanding it’s D.
