D’s comment above is correct. I just looked it up in my UCSPE. “Name”,”description”, and “prefix” is there in the first step but does not have a red box around “description” to show that it is required. The second step clearly says “Add IQN Blocks”.
The correct answers ARE C (prefix) and D (IQN Block)
