No it is both 1&2, (i.e. D). Raising a known error any time where it is USEFUL. i.e. where there is benefit to others in making people aware of the issue.
It seems to me that D would be the correct answer. However, I am trying to also confirm the answer as in
http://itilexamtest.com there are almost identical questions, Q31 which fives D as the answer, and Q441 which asks the same thing but with a typo that says C is the answer.
